Post-Hospital Recovery

The first weeks home are the ones that matter.

The week after a hospital stay is where most readmissions happen. Missed medications, falls, dehydration, infections caught too late. Our post-hospital recovery service is built to catch the small things before they become hospital things.

We coordinate with discharge planners and home-health teams so the handoff is clean. Then we send a caregiver who knows what to watch for, and how to keep the recovery on track without making the home feel like a hospital ward.

About our post-hospital recovery service.

  • How quickly can post-hospital care start?

    Often the same day. If we know about the discharge in advance, the caregiver can be at the home when your loved one arrives. With a few hours' notice we can almost always cover next-day starts. Just call.

  • What's typically included in the first few weeks after discharge?

    Daily or twice-daily visits covering medications, meals, mobility help, light housekeeping, and a watchful eye for warning signs (fever, confusion, swelling, withdrawal). We coordinate with visiting nurses and PT/OT so everyone is working from the same plan.

  • Can care taper off as my loved one recovers?

    Yes — that's almost always the trajectory. Most post-hospital clients start with intensive coverage and step down to a few hours a few days a week within 30 days. We'll suggest the changes; you decide.

  • Will the caregiver coordinate with our medical team?

    Absolutely. We keep a simple care log the family and visiting clinicians can review, and the caregiver communicates directly with the home-health nurse or therapist whenever they overlap.
